LVN Med/Surg – Nights

Scope/Impact

The LVN assumes responsibility for direct nursing care of assigned patients under the supervision of a registered nurse. The LVN utilizes knowledge of patient’s age and cultural diversity into the provision of patient care. The LVN also assists the professional nurse with assessment, planning, implementation, and evaluation of each assigned patient. The LVN contributes to the provision of quality nursing care through techniques that support and achieve positive outcomes in patient care

Job Functions

  1. Principle responsibility for the assisting the nursing staff in providing maximum patient care, assessment, planning, implementation, and evaluation of each assigned patient.
  2. Responsible for utilizing the nursing process, completes basic physical and psychosocial assessment of patients and takes nursing histories to provide a basis for development of patient care plans under the direct supervision of the licensed professional.
  • Using nursing process, plan and provides patient/family care; evaluates patient’s response to care provided and redirects care as necessary under the supervision of a Registered Nurse.
  1. Consults with a Registered Nurse to formulate care plans that reflect concerns for patient needs for the patient, family, and significant others.
  2. Reviews and updates the care plan as needed with the assistance of the Registered Nurse to reflect patient’s current status throughout hospitalization.
  3. Assesses patients at the beginning of shift for acuity of patient needs/problems and based on that assessment, established priority in which work activities are consistently completed by the end of the shift.
  • Gives exchange report to other health care personnel providing accurate and complete information on each patient at the change of shift, when transferring patient to another unit, and at intervals during shift.
  • Performs basic nursing skills competently to include but not limited to intake and output, vital signs, urine testing, specimens/cultures, dressing changes, personal hygiene for patient, skin care, glucose monitoring, airway/suctioning, basic oxygen administration, tube feedings, urinary catheterization, drains, and enemas.
  1. Recognizes variance of vital sign and assessments that should be reported to the Registered Nurse, supervisor, and the physician in time frame appropriate to the variance.
  2. Administers medications according to policy including IV’s and IVPB’s as prescribed by physicians.
  3. Maintain accurate and timely records according to hospital policy and procedures.
  • Performs all skills, functions, and responsibilities of a Licensed Vocational Nurse.
  • Demonstrates competence in the clinical skills required for patient care.
  • Involves the patient, family, and/or significant others in patient care and patient teaching.
  1. Seeks guidance as needed when performing nursing skills by utilizing resource persons, policy and procedure manuals, or others references on standards of care.
  • Maintains strict confidentiality of patients information at all times.
  1. The nurse will be expected to document all patient care in the electronic medical record. Patient assessments every shift , patient safety rounds every two hours, care plans updated daily, meals, vitals, I&O, and all other necessary documentation is to be documented each shift, correctly in the EMR, in collaboration with the charge nurse.
  2. Communicates with the nurse manager and the Chief Nursing Officer as appropriate.
  • Perform any other duties as required or assigned by supervisor.

License(s) / Certification(s) / Registration(s)

Required: Licensed Vocational Nurse currently licensed in the State of Texas.

Current BCLS, ACLS, (ACLS may obtain within 6 months of employment) and IV therapy skills.

Required: Graduate of an accredited School of Vocational Nursing.

High school diploma or equivalent
