HOME MEDICAL VISITS
Medically necessary home visits for homebound patients can be arranged through our primary care clinics. Services are provided by a CCMC Health System physician or nurse practitioner. Providers can also see patients in area nursing homes.
The Centers for Medicare & Medicaid Services (CMS) define the “homebound” patient as a person who cannot leave home due to a serious injury or illness that makes it difficult to move without the aid of a wheelchair or walker. Additional criteria must be met to receive medical home visits through a Medicare plan. Other health plans may also cover home medical visits and have their own criteria. Please consult your health plan for coverage details.
