Our Chronic Care Management program provides patients with two or more chronic conditions customized support and care coordination services. Chronic conditions include, but are not limited to, high blood pressure, congestive heart failure, chronic obstructive pulmonary disease (COPD), diabetes, cancer, and arthritis.

The program helps patients stay on track with managing their conditions, they receive support between doctor visits, and have better health outcomes.

To be eligible for the CCMC Chronic Care Management program, patient conditions need to have been diagnosed by a doctor and expected to last at least 12 months.
