Comanche County Medical Center (CCMC) is seeking an experienced Registered Nurse (RN) to serve as our Swing Bed Coordinator. This position plays an important role in coordinating the Swing Bed Program and helping patients successfully transition from acute care to continued skilled care and ultimately back home or to the appropriate level of care.

The ideal candidate is an organized, experienced RN who enjoys coordinating care, working closely with physicians and interdisciplinary teams, communicating with patients and families, and developing relationships with referral sources.

Key Responsibilities

  • Coordinate and oversee the Swing Bed Program, including the admission and referral process.
  • Review potential Swing Bed admissions for appropriateness and ensure admission requirements and documentation are completed.
  • Coordinate patient care and services throughout the Swing Bed stay.
  • Monitor patient progress and assist with ongoing discharge planning.
  • Work closely with physicians, nursing staff, therapy, patients, families, and other members of the healthcare team.
  • Coordinate discharge plans and communicate with outside agencies and post-acute care providers as needed.
  • Serve as a resource for patients and families regarding the Swing Bed Program and available services.
  • Maintain Swing Bed policies, procedures, program documentation, and required statistical information.
  • Assist with staff education related to the Swing Bed Program.
  • Support compliance with applicable regulatory and certification requirements.
  • Develop and maintain positive relationships with hospitals, physicians, healthcare providers, and other referral sources.
  • Promote awareness of CCMC’s Swing Bed and related services within the healthcare community.
  • Participate in quality improvement and reporting activities as assigned.
  • Assist with coordination of related outpatient services as needed.
  • Perform other duties as assigned.

Qualifications

Required:

  • Current Registered Nurse (RN) license in the State of Texas
  • Associate Degree in Nursing
  • Current BLS certification
  • Minimum of two years of hospital experience
  • Strong clinical judgment and organizational skills
  • Excellent verbal and written communication skills
  • Ability to effectively coordinate care among physicians, patients, families, nursing staff, therapy, and outside providers

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • Previous utilization review (UR), quality assurance (QA), case management, discharge planning, or Swing Bed experience
