Scope/Impact
Prepares patient records, processes requests for medical records information, files all test results, faxes, and loose medical records. Medical records clerk is in charge of Medical Records.
Job Functions
- Prepares patients records accurately by placing all proper name, drug allergy, and year labels to the file.
- Assures patient records are properly identified with date of birth for verification and attach proper reports.
- Processes inquiries regarding medical records, processes request for medical records, and assists with records release and copying, all in accordance with HIPAA standards.
- Supplies requested information to physician’s offices, insurance companies, etc. after verification of identification of requesting party.
- Prepares charts for upcoming appointments.
- Loads charts on a timely basis for all incoming messages, faxes, lab, x-ray or test results, or incoming physician referral notes,
- Loads all charts & loose filing in a timely manner as they come back into the medical records office.
- Answer telephones when Clinic Receptionists/Operator is out.
- Assist with scheduling, record keeping, and insurance processing when needed.
- Maintain proper level of supplies for the medical records area.
- Performs cashier dues when needed as backup.
- Must be able to effectively communicate with co-workers, staff, patients and visitors.
Education Required
Required: High school diploma or equivalent.
Preferred: Associates Degree in Health Information Management preferred.
Amount and Type of Experience
Required: Minimum of one year of experience in medical records office setting.
Preferred: Knowledge of medical terminology preferred.
